    Seems it is very easy to slip into exageration, even when trying to sound very even handed and “reasonable”.

    If you start with page 1 and read through to today, it reads like a fiction story. When I read some of the news, I thought to myself that if I read that info in a fiction novel I would put it down as too outlandish a plot.

    Today’s page carries a link to the latest warning from WHO for men to abstain from sex for three months. The fact has been reported for some time, WHO is just getting around to warning about it officially. While I would love to just trust the PTB, I think I will just continue to keep myself as informed as possible and err on the side of caution.

Filoviruses are able to infect via the respiratory route and are lethal at very low doses in experimental animal models, however the infectious dose is unknown. There is minimal information on how well filoviruses survive within aerosolized particles, and in certain media like the biofilm of sewage systems. Preliminary studies indicate that Ebola is aerostable in an enclosed controlled system in the dark and can survive for long periods in different liquid media and can also be recovered from plastic and glass surfaces at low temperatures for over 3 weeks (Piercy, et al., 2010).

    2.2.4.1. Topic: CBS-01, Determination and Understanding of Quantitative Infectious Dose
    This topic explores the physical, biological and molecular interactions of Ebola and related filoviruses to determine which of these interactions play a key role in the determination and understanding of quantitative infectious dose. The focus is on proposed efforts that aid in determining how many viable virions are required to cause human illness. Efforts that combine both experimental and computational techniques to explore molecular and physiological interactions and biochemical pathways using non-human primate models and a systems biology approach will be given priority.

    2.2.4.2. Topic: CBS-02, Persistence and decay or survival rates in the Environment
    The research should generate knowledge on Ebola persistence in the environment and knowledge to help predict potential mutations or changes to the viron. Studies should determine persistence and decay or survival rates of Ebola virus and other filoviruses in the environment. Research areas may include (but are not limited to):
    • Assessment under a range of controlled environmental conditions, including, but not limited to the dark, simulated solar radiation, and ranges of temperature and humidity.
    • Identification of environmental factors that contribute to persistence
    • Assessing persistence of Ebola on fomites/ surfaces after aerosolization
    • Assessing persistence in other media such as water, sewage biofilm, and other priority surfaces.

    2.2.4.3. Topic: CBS-03, Molecular Determinants for Persistence
    This topic focuses on understanding the molecular determinants that govern the ecology and environmental persistence of filoviruses. The research should focus on the molecular identities and mechanisms that promote environmental persistence of filoviruses. Research areas may include (but are not limited to):
    • Genomic and proteomic analyses to investigate the regulation of genes and proteins upon exposure to varying environmental conditions
    • Use of phylogenetics to identify potential genes involved in environmental persistence
    • Elucidation of mechanisms that contribute to environmental persistence
    • Metagenomic analysis of potential reservoir environments
    • Elucidation of mechanisms and environmental conditions that promote mutations in animal reservoirs”.

    But not warning about an Ebola pandemic that catches us by surprise has much higher costs. It’s not damned if you do and damned if you don’t. It’s darned if you do (warn) and damned if you don’t.

    And if our leaders don’t sound the alarm, somebody else will. People will start to find out or figure out that they have bigger Ebola problems than they faced in Dallas (and now face in New York). If our leaders aren’t the ones who tell them, they will not trust our leaders to guide them through it.

    I have really tried to avoid the “DOOM” sites and reports. I have really tried to understand my own personal weaknesses and frame of reference from my own life experiences. As a result I tend to “discount” my apprehension about ebola.

    There are a wide range of views here on Free Republic that I respect and use for perspective.

    But I have some questions. I would appreciate any response, from pancake bunny, on up.

    1. There is a major cry for stopping this at it’s source, Africa. Some examples follow:

    The WHO said this past week that there is no evidence the infection rate is dropping. A report in the Lancet medical journal said, based on a mathematical model of the outbreak in Liberia, that the U.S. military’s plans to create 1,700 new beds for Ebola patients is inadequate and that there is a “rapidly closing window of opportunity for controlling the outbreak and averting a catastrophic toll.” Liberia: Ebola Could Kill 90,000 in Liberia By Year-End Unless Efforts Scaled Up-Study

    London — Efforts to control the Ebola virus in Liberia must be quickly and dramatically scaled up or tens of thousands of people will die in the coming months, said a study published on Friday.

    Nearly 4,900 deaths have been recorded across West Africa since the virus was first detected in Guinea in March, according to the World Health Organization. Liberia has been the hardest hit of the countries most affected by the virus, with 2,705 deaths and 4,665 recorded cases.

    Without accelerated efforts to control the disease, Ebola will kill 90,000 in Liberia and infect 171,000 by mid-December, a study in medical journal The Lancet Infectious Diseases said.

    “Our predictions highlight the rapidly closing window of opportunity for controlling the outbreak and averting a catastrophic toll of new Ebola cases and deaths in the coming months”, Alison Galvani, one of the authors of the study, said in a statement.

    Additional medical facilities for Ebola treatment, a fivefold increase in detection of new cases, together with allocation of protective kits for households could avert as many as 98,000 cases by mid-December, the study said.

    Researchers based their analysis on a study of Liberia’s Montserrado County, which encompasses the capital Monrovia and where the vast majority of cases have been recorded.

    Without expanded efforts to control the epidemic, the national toll would be even higher, they said.

    DOES ANYONE THINK EBOLA WILL BE STOPPED IN AFRICA?

    Senior officials in China and India have been scrambling to prepare their countries’ medical systems to cope with possible cases. In India, top officials overseeing policy on health, civil aviation, shipping and other related issues met on Oct. 16 to coordinate plans. In China, the National Health and Family Planning Commission has called for medical institutions across the country to upgrade infection-control precautions by the end of this month.

    “The first thing at the top of their minds now is Ebola,” said Malik Peiris, director of the School of Public Health at the University of Hong Kong, after meeting on Friday with senior Chinese doctors and officials from the Chinese Center for Disease Control and Prevention.

    Dr. Peiris, who is best known as a leader in the fight against SARS, or severe acute respiratory syndrome, in 2003, said that flight and trade patterns between Asia and West Africa meant that five cities in the region would be at the front line in preventing Ebola from spreading: Beijing, Shanghai and Guangzhou in mainland China; Hong Kong, a semiautonomous Chinese territory; and Mumbai in India.

    The problem is the absolute statements being endlessly repeated. 21 days, fluid contact, not contagous unless symptomatic, etc. etc. are taken as absolutes. There will be exceptions.

    CDC has changed it’s fever guidelines. That wasn’t due to ebola mutating, that was a political decision.

    We are living in an analog world and decision makers are making digital decisions. What will happen when the “trip to Africa” has to be eliminated when we have “community” spread? That is the case in Africa now. Decisions made from a check list, depending on HONEST answers. Denial is a human condition, not a cultural one.

    The state health department just alerted us that the test result was negative for Ebola for the patient at Methodist University Hospital. We will continue to monitor and care for the patient. We proceeded with an abundance of caution today, and this experience makes our staff even better prepared should we face a case in the future.     That is a very interesting take. You seem to post as presenting yourself as a very knowledgeable, scientific, expert on health issues. Yet, you are happy to take other’s opinions about an article written by an Emergency Doctor than bother to spend no more than ten minutes to read it for yourself. Some read it and see the alarm about the flu, I read it and see the alarm about HCWs saying “Screw It”. Not to mention the various approaches to treating flu vs ebola that have not been mentioned in the comments here.

    Again, I apologize the manner that I posted this article has caused you so much anguish. Should you bother to actually read the article, you will see I simply excerpted the very first part of his article. It was his article and I showed him the respect of presenting the beginning of his article as he wrote it.

    I did not get the impressing he was trying to be “scary” and I did not selectively excerpt the article to try to be “scary”.

    While I realize there are a great number of “over the top” ebola items blazing around the internet, I thought this was an honest look at the issues facing the professionals that we are looking to for answers. Since I may be faced with the same type of decisions he is expecting to face, I guess I found some comfort in the article.

    I promise not to post any articles next Friday night. I wouldn’t want to be accused of trying to be “Scary”, Trick or Treat.

    Sounds like what New York is doing.

    The airports are covered but what about the situation in the hospitals?

    Are all the Doctors and Nurses and other HCWs that are currently taking care of patients in America going to have to go into quarantine now and stay in quarantine for 21 days after their patients are cured or die? What about the ones that were working on Pham? Are they all in quarantine?

    What is different about a HCW in Africa and one in America?

    We have a very angry nurse that was blindsided with these new regulations. How do you think Phams HCWs will react if they get slammed with quarantines??

    I do respect this audience, however I admit I did fail to realize the difficulty some would have with reading the article instead of having excerpted parts I found important spoon fed to them.

    As an example, I found the more interesting point not the concern for the flu. I thought the most important point was that he wasn’t sure they would show up for the party.

    Saw one of these Medical Experts answer that last night. To paraphrase:
    “We go to these hell holes and give a couple of months of our lives dealing with daily risk in extreme heat and deprivation. When we are able to come back home for the holidays and family visits, we don't want to be in isolation for several weeks. No one wants to put up with that. We are experts. We can self monitor.”
