A flare drum (you’ve probably seen flames coming from flares) had the wrong pipe covered with plastic (should have had a metal plate). The drum was connected to another plant that was still in operation.
If that plant had an upset that would have directed the explosive gases into the flare drum, which would have spewed out the visclean covered pipe and spread over the plant until it reached a sourco of ignition.
