No, you don't.

If you belong to one of these groups then you qualify for a religious exemption. You just have to make sure that they post the detailed explanation to verify that they've qualified as a religious organization under Obamacare.

As Alberta's Child confirmed, Christian medical-sharing ministries make one exempt from the O-"care" mandate, as if it is an insurance coverage. I have mixed feelings about this, but I suspect that some legislators wouldn't sign onto passing this monstrous socialized medicine bill without a carve-out for such ministries.

Actually, you don't even have to supply any proof, just affirm on the tax return that you have coverage.
