Czar is just new Latin for "caesar." The idea is the same, though, someone who dictates his will for others to carry out. A czar is way worse than a commissar. A commissar was the head of a government department in the USSR until 1946. He's answerable to someone or other in the bureaucracy above him. A czar is answerable to no one.
