Many posters say the nurse video is fake, but Iran does seem to be hard hit. Are Iranians somehow especially vulnerable right now? Have economic sanctions limited food imports to the point that Iranians are malnourished/lacking basic medicine? The great plagues of history with very high death rates usually came on the heels of several years of crop failures, wars, etc.
If we were ready, we would have enough masks and protective equipment for health care workers. Apparently, we're off by a factor of 10. And regular citizens are going to need masks too.
If we were ready we would have used the time we had while the virus was mainly confined to China to prepare enough tests for a large part of the population. Now the virus has spread widely and we don't have a clear picture of what is going on because hardly anyone can be tested.
If we were ready, we would have a plan for how we are going to deal with a vast increase in the number of people who need to be hospitalized.
