I've worked dozens of red meat and poultry plants, but I can't figure out what sort of device could have sucked someone into it head first, especially packaging equipment.
And everything under maintenance or being sanitized should have been "locked out/tagged out" before he got close to it.

Being the victim was working in a packaging area it could have been a banding/strapping machine. Those flat plastic straps that are wrapped around boxes for shipping. I also have worked a few slaughter houses and meat processing plants and can't think of a machine in a chicken plant that a person could be sucked into.
