Her book was published Hardcover – October 3, 2017 while her license was expired, but yet she is claiming to be an MD.

So, not obtaining or renewing a government required license invalidates the M.D.? That's like saying that failure to obtain or renew a government permit invalidates your second amendment rights!

“yet she is claiming to be an MD.”

She’s a moron, but she’s still an MD. MD or medical doctor is an academic degree confirmed by an accredited medical school which cannot be revoked after issued by the medical school(unless the institution determined that the degree was obtained via fraud, cheating which would be exceedingly rare). Of course medical licensure is subject to ongoing regulation which can result in expiration, reprimand, or revocation by the State.

She can opine on Trump and use her credentials to enhance her credibility with the public, but she is not practicing medicine (psychiatry is a branch) unless she actually sees the patient in person and does a physical exam.

All her statements are mere bloviating and have no legal or official standing. Even if she were licensed, her statements have no standing because she did not see the patient and conduct an exam.

A person would still be an MD despite an expired license. The MD is an academic degree, while the license is necessary for the practice of medicine. SO depending on what s/he is doing to earn a living now there might not be a problem here.
