A U.S. Navy warship made a sudden turn, colliding with a commercial vessel in Singapore territorial waters and resulting in the deaths of 10 U.S. sailors, Singapore's government said in a report Thursday.

A review of deadly ship collisions in the Asia-Pacific showed sailors were undertrained and overworked, Reuters reported.
The U.S. Navy subsequently announced a series of systemic reforms in hopes of restoring basic naval skills and alertness at sea. Just two days after the McCain crashed, Vice Adm. Joseph Aucoin was relieved of his duty as commander of the U.S. 7th Fleet.
The sudden turn by the McCain -- named for the grandfather and father of U.S. Sen. John S. McCain III, R-Ariz. -- was from a series of missteps at the control of the ship, said the report from Singapores Transport Safety Investigation Bureau, VOA News reported.

McCain? The crew was probably undertrained and over served.
Having served four years active in the US Navy, two west-pacs in the area of occurrence(s), I got a laugh out of this statement:
“A review of deadly ship collisions in the Asia-Pacific showed sailors were undertrained and overworked.”
In my four years of service I never met an “overworked” swabbie.
That said, my complete condolences to the sailors killed in both incidents.

Other accounts indicate that USN newbie junior officers spend too little time developing and perfecting basic seamanship skills and too much time on politically correct human relations course material designed to ease conditions for women, minorities, and trans types. My father, a Kings Point grad and former merchant marine and USN officer, was astonished from the start that such an accident could happen since the measures to avoid it are so simple and fundamental.
It happened before in a galaxy far far away.

The don’t, but they can. Saw a fully laden container ship dip a rail to avoid running over my net in the Strait of Juan de Fuca.
If I recall correctly, Lord Vader did not take that mishap well.
