Is it really that difficult to design a ship where passengers cannot inadvertently slip, accidentally fall, or be intentionally tossed overboard?

Ten-foot railings and barbed wire would be a real buzzkill.

Nobody falls overboard accidentally from those ships. They have good high railings. Most are suicides.

You have broken a cardinal rule. “Never underestimate the power of stupidity”.. if there’s a way someone can get by a mine field, barbedwire and a 20,000 volt electrified fence to fall over board. someone will do it. As a veteran, I can attest to the Navy saying,” make it sailor proof, and some dumb ass will still screw it up”

Not at all, and every cruise ship is designed so that passengers cannot inadvertently slip or accidentally fall overboard. You have to work at it to fall off a ship. Now intentionally tossed over the side? There's a limit to what any designer can do to prevent foul play.
