It's hard to understand all the verbal gender obfuscation which is used these days. But the story appears to be about a mentally ill girl, and her equally mentally disturbed mother, both of whom claim Kaysen is really a boy. Kaysen was born with a vagina and two X chromosomes. Nonetheless, Kaysen and her mental case mama claim she is really a male.

She’s female and prefers to be masculine rather then feminine. There are, also, males who prefer to be feminine rather then masculine. These preferences do not change them from one sex or gender to the other and neither does surgical mutilation.
