For diabetics and those on the Adkins diet, the smell is probably due to ketosis, a condition in which the breakdown of proteins for fuel generates ketones. Some people register the smell of ketones on the breath as similar to alcohol.

Apparently so. If a lot of yeast bacteria get in the gut, then a high-sugar diet can cause fermentation and produce a lot of alcohol.
