Menopausal hormone therapy linked to increased rate of dementia
Medical Xpress / British Medical Journal / The BMJ ^ | June 28, 2023 | Kejal Kantarci et al / Nelsan Pourhadi et al

Use of menopausal hormone therapy is associated with an increased rate of dementia and Alzheimer's disease, suggests a large study.

An increase was seen in long term users of menopausal hormone therapy, but also in short term users around the age of menopause (55 years or younger) as is currently recommended.

Menopausal hormone therapy (widely known as HRT) is used to relieve common menopausal symptoms such as hot flushes and night sweats. Treatments include tablets containing estrogen only, or a combination of estrogen and progestogen, as well as skin patches, gels and creams.

The effect of short term use of menopausal hormone therapy around the age of menopause, as is currently recommended, remains to be fully explored.

To try and fill these knowledge gaps, researchers in Denmark assessed the association between use of combined estrogen and progestin (synthetic progestogen) therapy and development of dementia according to type of hormone treatment, duration of use, and age at use.

The results show that, compared with people who had never used treatment, people who had received estrogen-progestin therapy had a 24% increased rate of developing all cause dementia and Alzheimer's disease, even in women who received treatment at the age of 55 years or younger.

Rates were higher with longer use, ranging from 21% for one year or less to 74% for more than 12 years of use.

The increased rate of dementia was similar between continuous (estrogen and progestin taken daily) and cyclic (daily estrogen with progestin taken 10–14 days a month) treatment regimens.

Use of progestin only therapy and vaginal estrogen only were not associated with the development of dementia.

This is an observational study, so they can't establish the cause, and the researchers were not able to isolate vascular dementia from other types of dementia.

Would the same thing be true of men with low T, who have been prescribed Testosterone Supplement for the rest of their lives? If this is not know now, it is likely to soon be investigated.

actually studies 30 years ago showed that estrogen post menopause led to a lower rate of dementia.

The side effects from high dose estrogen could make breast cancer and endometrial cancer grow faster, so we started to avoid using them. So high dose estrogen could cause blood clots and multi infarct dementia, so maybe this type of dementia is affecting the data.

On the other hand, estrogen keeps you nether regions young, and an active sex life means a younger happier old lady, and a happier hubby too.

Once again big pharma pushing crap without testing it properly. How did men and women survive before big pharma?

Fake news. This is synthetic hormones.

Bio-identical hormones are available and are state of the art. If bio-identical hormones caused dementia, it would be a disease of teenagers, not of old people.

Hormone restoration for men and women (with bio-identical hormones, not big-pharma chemicals) has a huge improvement in quality of life while REDUCING risk of age-related disease.

No! If T caused dementia (or prostate cancer, etc.), teenage boys would get it. The risk is with synthetic female hormones, and even that risk is exaggerated.

Testosterone restoration is excellent for quality of life and general health. The main risk with T replacement is that doctors will not prescribe enough to restore youthful levels. The significant negative side effect is potential for increased body hair and increased hair loss in men genetically predisposed to have male pattern baldness. Risk of hair loss (for a minority of men) is worth it, in my opinion, and it can be managed with hair replacement surgery if it bothers someone enough.

There may be risk if T is used in someone with active prostate cancer, but in that case, doctors will not prescribe it for fear of malpractice. Experts in T replacement might still prescribe it cautiously.

There are seven forms of dementia.

Alzheimer’s is one of them.

Ever work in a nursing home?

I’ve worked in three of them. Believe me I know what dementia is.

I lost my mother to it seventeen years ago.

My sympathies. My mother didn’t know who any of her seven children were in her final days.

She didn’t even recognize my father.
