I watch the Crime Channel and the Plane Crash Channel religiously (it keeps me off the streets). There's been at least one horrible crash caused by an improperly manufactured engine fan blade having exploded in flight. Actually...there was also an A380 that almost crashed because one of its engines (Rolls Royce) exploded in flight. They say that it was only luck and an incredibly skilled cockpit crew that prevented it from going down.
