The original Scorpions "Virgin Killer" album might be the worst record cover of all time. Even going by the skewed sexual morality of the 1970s, it way crossed the line.
I remember when it could be found in the import section at record stores here in the U.S. The much more nondescript replacement cover would be in the regular section.
