First 3/4: A (flimsy) shack in the mountain. A mother and her children live there. The video shows the wretched living condition of the family.
Last 1/4: Privileged N. Koreans are throwing a send-off party for a boy who is leaving for 10-year military service. It was held in an upscale (Chinese) barbecue restaurant in China. Karaoke machine, barbecue on a grill can be seen, and people are having a good time.

In this case, the communist regime is responsible for it, the one who pour in perhaps hundred of millions of dollars to its nuke program, the money which could have been spent to feed these folks.
As you said, this kind of wealth gap is not unique to it. Just the way to cause it, the extent of the gap, and this is all due to a conscious decision made by Kim Jong-il who knew its consequence and see it as a positive measure to eliminate "disloyal class."
