That one out in 2004 came after we [Yeah, I’m a Yankees fan] blew a 3 game to zip lead in the League Championship against Boston.And since then, we haven’t been able to take 3 out of 5. Sure, the players are at fault, too. But so is Joe, and he got a pass WAY too long [since 2001].

Actually it was before the "meltdown", and occurred in Game 4 in the bottom of the 9th Inning (sorry, I said "one out away" when it should have been "one inning away")

Yanks had a 4-3 lead going into the bottom of the 9th, and were poised to sweep the ALCS in 4 straight with Mariano on the mound, if only .... if only:

Rivera walked Kevin Millar to begin the inning. That simple base-on-balls would prove to be the turning point of the series. Dave Roberts pinch-ran for Millar. With the Red Sox down to their final three outs, Rivera checked Roberts at first base several times before throwing a pitch to Bill Mueller. On Rivera's first pitch to Bill Mueller, the speedy Roberts stole second, putting himself in scoring position. Mueller's single allowed Roberts to score, resulting in Rivera blowing the save and the game going into extra innings, tied 4-4.

As we all know, Boston won that game in extra innings and then went on to win the next three to complete one of the most humiliating (for the Yankees and us fans) defeats in all of modern sports history.
