Around here (Atlanta GA metro) they come cut in half on the diagonal, and I just hold it like a regular sandwich and BITE.
Is there some sort of protocol of which I am unaware?
You have to widen your stance and sort of bend over so that all the mess doesn’t fall on you. Otherwise just enjoy.

You eat it with gusto and without pinkie fingers in the air. And if you do it right, grease is everywhere but on your clothes.

Well..I just flip mine on the side and take a bite on the side with the toppings to sort of cut it down to size, then the next bite on the cheesesteak itself.
That *is* how one eats a Philly cheesesteak. No unusual protocols.
Of course, I hadn’t realized that Frieda’s lunch truck outside the Penn Math Dept. was such a class joint. Frieda always used slices of provolone as the default source of cheese.
My favorite was the variation on the theme, the pizza-steak: a cheesesteak with a few spoonsful of pizza sauce tossed on late in the cooking process after the cheese has started to melt properly.
