The difference is that the physicians know that the test serves little medical purpose, but that the results thereof will serve as the pretext for an honour killing. This makes them complicit, if not legally, certainly morally.

Exactly right. Plus, ask any gynecologist if a virgin can have a broken hymen. The answer is yes. Fall off a bike, ride a horse, fall down some stairs, etc... and it is common.
