Yes. In my boiler days, we used x-ray, magnetic flux, and ultrasound NDT. That was in the factory as well as in the field. It is incredible (more like inconceivable) that this got by their engineering and QC teams.
It kind of made my spider senses tingle reading it (patents and engineering in NDT). They are hiding something.
Tinfoil hat time. I wonder if there was any Roscosmos consultants on SpaceEx payroll.

...including all nine Merlin 1D engines, operated nominally... How does an engine work nominally?

Minimum wage. 9 to 5. Bagged lunch. Regular oil changes.

I was thinking sabotage from the moment this happened. There are many who would like to see SpaceX fail, or at least slow it down.

One Pilot was killed, the second one survived.
Virgin Space Vehicle designed by Dick Rattan..

I agree. I was impressed with the business plan using standardization and assembly line production. I think they are hiding something.

In addition, the Dragon spacecraft not only survived the second stage event, but also continued to communicate until the vehicle dropped below the horizon and out of range.Couldn't someone have sent a "Deploy Chutes" command to the Dragon?

I agree. Considering it failed right at maximum dynamic loading, supposedly 1/5 the strut’s capacity, means they may not be telling the whole story. The vehicle/payload may have had harmonic vibration issues which they did not calculate or predict. Ergo, a systemic program failure.

I’m wondering the same thing myself.

It stays within a certain burn/thrust profile.
