The recreational use of alcohol does have cumulative terminal effects, and the people who use it that way should be denied health insurance.

I'm OK with legalizing drugs for recreational use as long as the drugs are required to contain ingredients that will have cumulative "terminal effects" on the user over a relatively short span of use - say, 1 week.

Does that include the recreational drug alcohol?

The recreational use of alcohol does have cumulative terminal effects,

Should it be required to contain ingredients that will accelerate the terminal effects to 1 week, as you advocate for other recreational drugs?

and the people who use it that way should be denied health insurance.

Insurers should be free to so decide (or decide not).
