More likely some idiot using his engine wrench on the oxygen system rather than using a clean one. Mixing pure oxygen and flammable substance can cause a problem, as in

I saw an article last year that most aircraft maintenance is contracted out, and that the contractors are often bottom feeders who use Mexican (and other foreign nationals) as "mechanics" and "technicians". It used to be that you had to have military training, and then go through the airlines own tech academy. Not any more. Those days are gone with the wind.

Aircraft skin can be damaged if you don’t use special alloy tools. Some metals will rapidly corode if scratched by certain metals. Hard to in aging a certified aircraft mechanic making this kind of mistake.
