The U.S. Navy has removed the two senior officers and the senior enlisted sailor on a U.S. warship that almost sank off the coast of Japan in June after it was struck by a Philippine container ship, the Navy said on Friday.
Multiple investigations have yet to apportion blame for the accident that killed seven U.S. sailors aboard the guided missile destroyer the USS Fitzgerald.
However, the punishments are the first public admission by the U.S. Navy that mistakes by the crew contributed to the deadliest incident on a U.S. warship since Islamist extremists bombed the USS Cole in Yemen's Aden harbor in 2000.

Maybe he was in charge of lookout or bridgewatch?
A massive cargo ship sees you, and signals your vessel - and NO ONE on an advanced Navy destroyer sees it or responds?

my guess is that this person neglected his duties in some way....an accident like this one will probably ruin all but the juniorist of officer and enlisted mans career in the navy. the captain will never get to have the responsibility of a ship again even as a civilian ship captain.
my brothers a ships Pilot for the Alaskan state fiery system second in command. he told me one mistake and it is over for a captain his second along with any officer on the bridge of a ship that runs aground or hits another ship on open seas. no such thing as a learning experience with an accident.

The senior enlisted was the Command Master Chief.
Anyway this information is nearly a week old.
I think the senior enlisted is (jointly) responsible for training, discipline, and welfare of the crew. He would perhaps be one of the ship’s crew with the widest experience and longest service. He would be the enlisted crew’s chief representative. If the senior enlisted crewman were not doing their duty, it would show in the crew’s performance and discipline.
It’s almost like science fiction. I don’t understand it.
It would probably require at least 10 people neglecting their duties.
Never let a crisis go to waste:
immediately suspend all training in the military regarding gender roles and sexual harassment.
I guess they don’t post lookouts like they used to. Too expensive. Plus it’s cold and nasty on the bridge wings sometimes.
I thought I read their should be at least half a dozen.
I blame it on cell phones.
Bet those Officers did great in transgender sensitivity training.
When I was in the USAF, they did not consider any accident, to be an accident. They were all caused by something, whether mechanical problems, or human error, or other contributing factors, sometimes beyond the ability of people to save the aircraft. I suspect the Navy is the same way. I doubt they believe in "accidents" either. Usually, it's a combination of factors.

Would this affect their ability to buy a boat and become commercial fishermen? ;)
Are they allowed to text on duty?
