It starts with The Still Report. Bill Still says that during the Commander-In-Chief Forum on September 7, 2016, moderator Matt Lauer went “off script” and asked Hillary about her using an illegal, private email-server when she was secretary of state. Still's source is an unnamed “NBC associate producer of the forum.” It's a bit curious that an NBC producer would spill the beans on her insane meltdown.
The details of her insane meltdown as revealed by the NBC producer are interesting and scary. I vaguely recall this being reported when it happened. See above link.
