Per the 2016 NICS OPERATIONS REPORT.

13 - Full point of contact: Contact state/territory for all firearms background checks including permits (FL, HI, OR, CA, NV, UT, CO, IL, TN, VA, PA, CT, NJ),

4 - Partial POC: Contact state for handgun and FBI for long gun background checks (WA, WI, NH, MD)

3 - Partial POC: Contact state for handgun permit and FBI for long gun background checks (NE, IA, NC)

36 Non-POC: Contact FBI for all firearm background checks. Remainder of country including N. Mariana Islands, Guam, American Samoa, VI and Puerto Rico

2016 FEDERAL DENIALS- TOTAL: 120,497

Convicted of a crime punishable by more than one year or a misdemeanor punishable by more than two years: 49,510

Fugitive from justice: 24,502

Controlled substance: 11,278

Misdemeanor crime of domestic violence conviction: 8,854

State Prohibit: 7,686

Under indictment: 6,366 Adjudicated mental health: 5,638

Remainder: 6,663

I believe you fill out the info on the Form 4453 before the NICS phone call. Dealer keeps the form, Leo’s come and get the info and go from there. They ought to think that if ineligible persons fail to make the purchase they’ll try elsewhere.

how many of those were ultimately reversed due to error?

as an FFL, every denial I ever had on an Instacheck was a mistaken identity or some other type of bureaucratic foul-up ...
