Sounds like they used post tensioned not pre tensioned.

Yep, I see a lot of post-tensioned designs in residential homes' slabs. I'm afraid that many contractors are using that design to cut back on the rebar and the concrete thickness.

When doing inspections, I see many residential post tension slabs with the ends rusted off. Even the anchor plate is gone.

They put too much sand in the forms and pull the cables out of the concrete into the sand. I’ve seen foundations where the concrete is less than an inch thick in places.
