The facts within the medical record (test #’s, images, lab values) belong to the patient. But practitioners write subjective notes, assessments, recommendations and observations. THOSE belong to the clinician.

Ask for your records and you will get the technical facts with the clinician’s SOAP notes redacted.

Otherwise, clinicians have been sued for observing ‘patient is fat’; ‘patient is passive-aggressive’, et al.
