It can’t be fixed as long as income is taxed.
Too many people get income from sources other than W2 employers.
The most efficient approach would be a national sales tax with simultaneous elimination of all income taxes.
Most states, 48 I think, already have the collection mechanism in place.
No one will ever have to report how much they earn and no more filing.
