Your first impression is close to correct. Drug trade from Mexico, Columbia, China, etc plays a very minor role.
Doctors prescribe because they were mandated to prescribe, or lose all federal funding ... 45% of their money. and lose their effective license, especially the licenses of hospitals, nursing homes (that have a greatly increased number of non-elderly addicts on SSI/SSDI) and urban and rural health clinics and labs and on and on.
The swamp created and perpetuates this.
