Hole in Soyuz spacecraft found on ISS was 'made in Russia'

Several days ago it was reported that astronauts found a hole in the Russian segment of the International Space Station that was leaking air. One of the astronauts plugged the hole with his own finger before sealing the hole with a nylon ribbon to stabilise pressure on board the ISS. The hole posed no threat to the crew, but it had to be fixed.

According to updated information, the 2-millimeter (0.08-inch) hole appeared in the spaceship when the Soyuz was being built. Reportedly, an employee made a mistake, but it was decided to fix the problem with the help of glue. When it space, the glue plug reduced in size and popped out.

From experience, My Company hired software developers in Moscow to work on out flagship systems, only to discover that they left bugs ( not intentional spy-ware but just crap) which tool years to find and fix.

Humans make mistakes. I was trying to ventilate my attic once and drilled right through my roof shingles. Thankfully the roofers were still there (new build) and fixed it in about 5 minutes.

The REAL QUESTION is whether the guys in the shop are taught to SPEAK UP and get the right people looking at a mistake, or something weird - or are they too scared to do that. Had the (informal) ‘field fix’ held (in this case), we wouldn’t be reading about it. But the people making the fix were likely not qualified to consider the entire life cycle of that fix, so their fix didn’t make it long-term.

The key is having an environment where people are NEVER afraid to speak up if they do something wrong, or see something wrong. In the category of “we’re not much better” A little known tidbit that I found on the web, years ago, looking up Space Shuttle systems (since removed) - the launch right after the Columbia explosion (which killed 7) nearly had the same outcome. Seems that half of their pyrotechnic initiators were missing o-rings. Nearly caused the huge External Tank to NOT separate after launch, leading to a non-nominal outcome (i.e., dead crew). The techs knew something was funny - some had o-rings, some didn’t, but some career-climbing Leftist (probably) had them too scared to SPEAK UP and point out the likely issue.
