They should test it on all the patients, not just those who are the sickest. Healthcare workers are using it as a prophylaxis.

Yes. It seems counter intuitive to say that but it is exactly right. Relatively healthy individuals infected will rapidly improve and will be most able to resist any negative side effects. Stupid to wait until a last resort for an extremely compromised person with advanced stages on ventilators. Those patients will see less improvement and also be too weak to resist the side effects to this drug the worst being arrithma.
