The patient and doctor simply sit down, talk about HCQ. If they both agree that or would be beneficial to the patient, then the doctor writes a prescription. Done.

Maybe in some states, not in all
If you are going to get sick, try to choose a provider in a state that hasnt put politics ahead of science ( the science of using a drug that doesnt care what disease you have, just about moving zinc into virus cells and killing them)
