Not yet but it’s constantly being reported that it’s about to. That’s been said for years. And yet the bloody thing is still there. Makes one wonder.

nope

other smaller older ones did along the way

lots and lots of flooding did occur even without 3gd failing

The You tube poster thinks there is erosion under the base of the dam. This has been speculated for some time.

Somewhere I read that as engineered, they did not provide a way to sluice out sediment/silt building up behind the dam. I would assume—I am not an engineer—that as time passes there is increasing silt build up, and the total volume of water that can be held by the catchment is reduced.

Still waiting to see what happens, I guess.
