You know those small grocery store plastic chips with barcodes on them? Since there are a LOT of vaccinations for stuff, it wouldn’t be a bad idea, especially if you are traveling to Africa or something. QR codes can hold a lot of data.
You might even use both sides of the chip, with one side holding your vaccination record and drug prescriptions, and the other side with your medical history.
