1. torque wrench improper use.
2. torque wrench calibration overlooked.
3. torque wrench not used, due to improper training.
4. Improper QA procedure.
5. QA penciled.

As former DoD QA at DPRO/NAVPRO faccility, did witness These sort of things.

Did you mean QA was falsified? The way Biden falsifies everything about his life? The way that Claudine Gay falsified her research?
