Is this terrorism or is this lax safety protocols? Poor workmanship?

A wheel coming off sounds like lousy maintenance. We're certainly hearing about these issues more than usual the last month or so since the Alaska 737 plug door fell off - but are they happening more than usual?

From what I've read passenger jet tires are replaced about every 3 months or so depending on flight cycles. A triple 7 with 14 tires may cost well over a million dollars every 3 months to replace the tires..
