I don't think the questions and hard decisions are any different today than they ever were. Technology allows us to keep people "alive" for long periods of time, but the clinical and natural definition of death has not changed.
The Catholic Church has it right on this one. When a person is in a condition where the prognosis for recovery is poor and cannot survive without the assistance of a ventilator, then the natural course of events that constitutes "death" by any objective measure is well underway.
