How are bishops chosen now? Do they work their way up in the church bureaucracy? Are they appointed by a Pope?

Whats the difference between a bishop and an archbishop?

And how do some bishops or archbishops get designated as cardinals in the church?

Is there a systematic problem with how people are recruited and promoted in the church?
