No other state has such an abysmal percentage.

I think the percentage may be even higher. Personal anecdote. My 81 yo mother with dementia passed from COVID at the end of April. Her assisted living community packed her off to one of the local hospitals where she was tested, diagnosed with COVID-19 and died within about 12 hours.
I called the administrator to pass along the word and he told me a second female resident — my Mother’s ‘nextdoor neighbor’ also passed from COVID. That’s 2 COVID deaths that I know of for sure from that facility. BUT that facility reported ZERO COVID deaths to the State of Pennsylvania as of July. The only way they could get away with this is if they are shipping symptomatic patients out to the hospital to die. So if enough assisted living communities are playing this game the COVID reported death-toll for nursing facilities should be much higher.
