True, I got off an exit on the Jersey turnpike to go to the bathroom a few years back. I had to avoid the mold, step over the leaks, while fighting off the bugs so my kid could piss. Most disgusting bathroom I have been in. I hoped it was just dumb luck that I used this gas station as is a one-off uncared for bathroom but it may not have been.
I went to one recently in New York. Fortunately I had toilet paper in my truck because the loser that owned the place told me it was too expensive and he didn’t supply it.
I think they just resemble the bathrooms in the home countries of the workers; often they just say they’re broken (so you never see it).
