The surgery he is doing is very risky and only a few people can do it. There is a big risk of infection.

If it doesn't work, than he can go for lipo suction.

I have a cousin who got a second chance at life thru this surgery. She went from a being 5'7 weighing 340 lbs to 135 lbs. Night and freaking day, not only in body, but most importantly mind.

He is truly pathetic if he has only lost 75 pounds in one year with the stomach surgery. Most obese people lose that in half the time. I have a friend who was about the same weight, but 5-6 inches taller....he is doing cardio twice a day, eating right & working out....he has lost 40 pounds in 2 months.
