The text in the background is Dutch and says something like “this is a non-negotiable document”. It must be a check or a stock certificate. (Dutch is not the official language of England or Kenya).

Wellllll -— it COULD be that the government agency bought forms, or the stock for the forms, from the low bidder, in this case a Dutch company. Not bloody likely, though. Just possible.

Why is the signature of the attending physician and signature of physician different?

I read the Doctor's name as Kim. Under attending physician it looks like DM after the Dr.'s name. It's hard to read the first name.

Of course it could be. You just have to take the time to properly research this sort of thing. Like the time I clicked through on one of those “truth about colon cleansing” advertisements. Man, did I ever learn a lot! Did you ever try that? I mean, the kind of stuff that works its way up there I guess while you’re asleep or something (man, I gotta wash the sheets more often) like tapeworms and the kind of stuff that you usually only see on late-night sci-fi movies bursting out of somebody’s stomach, which I sure wouldn’t want to happen to me because then I really would have to wash the sheets. But you can get rid of it, see? And you don’t have to pay some 400-lb nurse named Agnes to stick a 9-foot tube up yer butt, either. It’s all natural! I’d be happy to tell you all about it but Jim Thompson doesn’t like product advertisements in his threads, so if you’d just post your credit card number, expiration date, and that funny little security number on the back you could have this stuff in your mailbox by next Tuesday. If the package is leaking a little, it’s OK. Oh, if you put “CONFIDENTIAL” on your FR posting nobody else will be able to see it.

If it is Afrikaans, it might just be that Kenya was using a type of unalterable paper from another available African source - and a Commonwealth country.

Again, this is for a document authenticator. This feature might be a dead giveaway of a forgery, or - it might just be a detail that is authentic.
