If I were to go out and purchase a non-subsidized policy, the cheapest appears to be a premium of $256/month (age 58, male, non-smoking) with a maximum benefit payout of $100,000/yr (not much by modern standards), but a very small deductible. Prescription coverage IS included. The quote for my wife is about the same. This is in Washington State. So for both of us the annual premium would be about half the 12k.
