Let's see how high they are on intrusive questions after that!I just read an article about how some parents did exactly that to the teachers who were giving their children equally intrusive questionnaires. When the teachers objected, the parents just indicated that the questions were no more inappropriate than the ones the schools were asking their children.
However, I would bet the school bureacracy missed the point. After all, *they* are the ones who know what's best for children. /sarcasm

I suspect that "SIGNA PPO" is actually CIGNA PPO, a managed care company that I loath and dropped out of when I could not ethically practice according to their inhumane requirements. I had many a ferocious argument with the sociopaths who are in Cigna's main office.
I also strongly suspect that the questionaire is issued by Cigna who mandates its administration to all teenagers. I wouldn't trust these b*st*rds with information about the price of tea in China, much less my child'd intimate secrets.

I'm glad I read this! I'm taking my 14 yr. old for her physical tomorrow. I'll be sure to alert her in case the practice tries to get her to answer any questions like this.

That's the same kinda crap my college wanted me to answer plus really explict sex questions (like "how many men have you slept with?")
I threw the form in the trash.

Everywhere you go now you hear the excuse "We have to do this because the state law says so." Sad but true, several years ago I went to write a check in an auto parts store, the young guy behind the counter pushed an ink pad at me, I said what is that for, he said he needed my thumb print. When I got done yelling at him, he told me that it was state law, ooops, the yelling started again.

Alright! No, it does not take a village to raise a child, it takes loving, available parents, well informed parents in this day and age, it takes wisdom and putting doctors in their place. Doctors are being seduced to become informants, why is it that they think they will escape becoming slaves?

File a "boundry violation" complaint against the doctor. Use that specific term and file the complaint (in writting with a lawyer's letterhead) with the state medical board, or other regulatory agency. Send it 'return receipt requested'. Better yet, have a lawyer file it for you.

Do you drink? When, how often?
Do you use drugs?
Have you ever been accused of malpractice?
Have you ever been sued for malpractice?
How much malpractice insurance do you carry?
Have you had any homosexual excounters?
Have you seen a prostitute in the last 6 months?
Do you beat your wife/husband/girlfriend/boyfriend?
