good girl! mine too ( the doctors had to dress me up in scrubs and have me carry my daughter into the operating room when she had her last surgery- I could not help crying when they gassed her and she went out...)

It's so heartbreaking when our kids are sick. In my opinion, any doctor that WONT let me go in the office with my child, is waving a very large red flag. No thanks!! We will find another doctor who will let me stay with my babies/children.

Some of the info MIGHT BE relevant to the Medical Care of the Child; NONE of the Info requested is ANY Business of "The State" UNLESS there is evidence of a CLEAR THREAT to the Child or her Siblings (IMHO!)

Absent CLEAR INDICATIONS that the child in question is at risk in her current environment, the State has NO Business gaining access to her Medical Records.

The "Alleged Questionnaire" is likely to be a "research tool" for some University Graduate Student--it is NOT a legitimate "Vector" for information normally available to a Pediatrician's Office!
