This questionnaire asks questions like:
Have you ever been hit or injured?
Is there any conflict of fighting in your home?
Have you ever had sex?
Do you or Does anyone you know drive after drinking or using drugs or have you ever ridden with a driver who has used drugs or alcohol?
Have you ever had unwanted sexual contact?
Is there a gun in your home?
Do you or anyone you know ever carry a weapon?
The questionnaire starts out by saying that the youth doesn't have to tell their parents about this survey and there is a check box the child can check as to whether or not their parents will be told!
BE VERY CAREFUL ABOUT WHAT YOUR CHILD IS EXPOSED TO WHEN GOING TO THE DOCTOR!
This is completely out of line and the medical profession needs to be put in their place on this one!

What are the odds that the doc is being paid to help out with somebody's research study, and it blew up? Much lower than the odds that someone, somewhere in government decided this was a neat way to snoop on parents by interrogating innocent children without adult guidance and certainly without legal counsel.

Well don't go to this doctor any more. Questions pertaining to possible health related issues, even sexual related ones are sometimes necessary; Especially by SOME 13 year olds. If its too intrusive don't use that person unless your looking for the legal lotto...

That list of questions sounds familiar. I've seen another (maybe more than one other) account about the same or similar questionaires being handed out by pediatricians, at schools, what have you. I'm not sure that the state government was directly involved.

As I say, neo-Puritanical Liberalism and Islamic fundamentalism are very much alike. Liberals would love to establish the Taliban in America--under their control. In fact, like the Talibani, they want to control everything. Everywhere you go now you hear the excuse "We have to do this because the state law says so."
This goes as far as mandatory tips on parties of 6 or more at a restaurant. I was told that the state law dictated that sales tax was to be collected on tips too if the tip was part of the total bill.
As citizens I think we have to challenge anyone and everyone to produce the written law they are citing right on the spot. If they can't comply with the law they're enforcing in writing, then we don't comply. Tell them go to hell.
Time for the sheeple to scream.

I know -- next time around, have your own set of questions for the M.D. and his nurse asking them about their own sexual life experiences, sexual preference, experimentation with drugs, alcohol intake, etc., etc., etc. You can't be too picky about your child's doctor. Oh yes -- do they have a gun in the house?

doctors have every right to offer sexual advice, birth control, etc. without parental permission. And I have every right to punch the socialist busybody in the nose and tell them it's none of their g*@ d#@%&d business.

You know, it's not just children. This may sound weird, but I'm 42 years old and I went to my doctor about a stomach problem. He gave me a questionaire all about sex! It was so weird. I wouldn't even write what these questions asked, but I told him that I didn't understand what this had to do with my stomach ache. He implied that many women have ailments that are caused by sexual frustration! I kid you not. I didn't fill out the form and never went back to that quack.

Time to turn the tables - use a liberal play - lawsuit city!
I know it's irrational - but maybe parents should start doing something about this crap. Scream bloody murder! Call the news papers / TV stations. Sling completely false and racial tinted accusations. When you get on camera, cry. Tell the world that your precious child was done irreparable harm by this obviously biased (all kids - ?), institutional discrimination.
Oh, sorry you are in one of those states that has already sold your soul for the collective good.
I apologize for the sarcasm. I would have had several veins burst by now had this been my baby girl.
Anyone catch the Shawn Hannity show today the Mom calling in about what the teacher said to her 9 yr old son for what he wrote?
Talk about institutionalized discrimination we, as conservatives, are on the receiving end.
