Psychologists and physicians explain this phenomenon in quite different ways; but a successful psychotherapeutic or pharmaceutical therapy is not known. Lobbies of persons suffering from BIID explain the desire for amputation in analogy to the desire of transsexuals for surgical sex reassignment.
It is simple mental illness exasperated by modern drugs forced onto people by unethical doctors who are paid by big pharma.

What they want is to be taken care of like a baby. They are fixated in the area of development where they were happiest. This is probably a failure of parenting: many parents have sufficient wit to take care of a small baby, make sure it’s fed and diapered, etc, but once the child is walking and talking, the immature parent grows bored with it and hands it an iPad or something. So the only nurturing, cuddling love they got was up to about the 18-month mark. So that’s where they want to stay: helpless and dependent, because that kind of care is the only love they’ve ever known.
