The subsidies are based on your income, not your medical requirements. All you have to do is accurately predict your income for next year. How hard is that? (Don’t answer!)

My brother owns a construction company. Health insurance per employee has gone up $200.00/ month per employee and deductible went up to $10k per year. So much for being affordable. If the stupid among us get hammered by the IRS, good!
The IRS will not go after these people—it would lead to too much bad press for the Wons’ big plans for a single payer system.
