In the USAF, there used to be a rule: if you fix something critical in the plane, you get on board for the next test flight. That way, the person who does the fixing has a personal stake in the quality of the fix. I don't know if that's still true in the USAF, but it certainly is not the case in government. They are completely walled off from the consequences of their decisions.
