If he has renal failure the only thing that can help him is dialysis and getting him in a hospital with a dialysis unit is the most important thing you can do - besides cleaning the poisons out of the blood, it can also remove excess liquids. The amount of liquids to be removed are determined by what his nephrologist would establish as ‘dry’ weight and the weight each time he gets dialysis.
My kidneys failed back on Memorial Day weekend and I had to go ER, and the hospital had a dialysis unit. Been on it ever since.
To me it’s an unnerving process and first seems like a never-ending purgatory, but it gets better. The ladies at the dialysis clinic I go to now are very good, caring and dedicated - I consider them as Angels. I wish your brother the best of luck and treatment.

OMG !!! My Father died from Wegener’s Granulomatosis back in ‘99. We had to get a Doc from Mexico to diagnose Him. I have heard of Survivors since then including a Doc the My Aunt worked with. I don’t know what the current protocol is for it, but there’s hope now. Hang in there and Contact the WSGS Group for more information.

Thank you from the bottom of my heart for your prayers and encouragement. As you personally have experienced this illness, a little more information for you.
Looking at the original post, my timeline is a little off, because time is both rushing past and dragging for me simultaneously.
He survived an earlier encounter with this. He was hospitalized for three weeks and had his own “Dr. House” style diagnostic team working on him. They figured out a treatment regime that stopped and considerably reversed the damage and he was actually in remission for almost 9 years.
Then about 7-8 months ago something was *off*, and he very gradually (much more gradually this time than before) started showing tell-tale symptoms, but their timing was so slow as to be barely noticeable.
He went to his physician and told her what he thought was going on, but she was skeptical until he convinced her to order a specific series of tests. She reluctantly did so, then admitted he was correct when the results arrived.
At first the latest iteration of treatments seemed to be working, and the “tapering off” stage began. Then the literal decay in his condition started. I took him to local emerg at 2pm on Sunday.
They moved him to a larger centre that night, and the second hospital sent him to the largest centre in the region yesterday afternoon around 3pm immediately following our visit.
Odd thing: his kidneys were working at 120+% for several weeks, then their function catastrophically collapsed, and his blood pressure fell to the basement (worst reading I saw was 65/37, last I saw was 94/62). This is in combination with the buildup of fluid in his lungs from the kidneys not doing their job anymore.
