That's my take also. Nothing overtly nefarious, other than a Government Bureaucracy that breeds and tolerates incompetence and complacency (complacent since there is no accountability).
From what I've read, so far, I blame the sloppy button pusher less than I do the people who designed their system and the management that signed off on a system that did not require double-checks.

Local news said the unnamed employee hit the button twice. I would’ve also. Once for the initial mistake. The second time would’ve been the “oh sheet!” instinct in an effort to “turn it off” (even though it wouldn’t have).
